Use your own gauge and read the numbers in the book tables to create your own glove.
Frustratingly and embarrassingly, it looks like the Index Finger instructions got lost. This is what should be there.
Index Finger
Return remaining held sts to your needles. With RS facing, rejoin yarn and pick up and knit the Pick up & Cast on number of sts at the base of the Middle finger, over the cast on stitch(es).
Your total number of live stitches for the index finger should be 2 x Index finger number + Pick up & Cast on number. For example, if you have 58 stitches for the Hand, you should have 18 stitches for the ring finger: the Index finger number in this case is 8, and the Pick up & Cast on number is 2.
2 x 8 + 2 = 18.
Distribute sts across needles as you prefer and join for working in the round. Finish as per Little finger.
