Warning: The cables on this pattern make it REALLY snug. Pretty much all of my testers had to start over with a larger number of stitches. A cast-on of 76 or 80 stitches would not be unreasonable for a larger foot/ankle.

The cables are easy to memorize in this pattern. The gusset increases are centered on the back of the heel. No heel flap, and no picking up stitches.
