Pattern Errata - 10/15/15:
For the Jimmy Beans Wool version only. The Button-hole row for the 3-Button version should read as follows:

k4, bind off 3rd stitch by slipping it over the 4th st, k5, BO 4th st by slipping it over the 5th st, k5, bo 4th st by slipping it over the 5th st, k 1.
